PUPPY PACKAGE TERMS AND CONDITIONS

1. Non-wellness exams, diagnostics, treatments, and medications required due to illness, injury, or hereditary problems are not included in the Puppy Package and payment in full is due at the time of service.

2. Visits should be scheduled 3 to 4 weeks apart to maintain the proper vaccination schedule. If additional vaccinations and exams are required due to un-scheduled or un-kept appointments, there will be an additional charge as these additional vaccines and exams are not included in the Puppy Package.

3. For small to medium breed dogs, the neuter surgery can be performed between 6 and 9 months of age. For large breed dogs, we recommend that neutering is delayed until 12 to 18 months of age (an additional fee of $110 will apply to dogs weighing >75lbs due to added costs associated with increased patient size). There will also be an additional fee ($355 to $1030) for puppies with one or both undescended testicles, a condition known as cryptorchidism). (Fees listed are an estimate of cost and are subject to change.)

4. For small to medium breed dogs, the spay surgery can be performed between 6 and 9 months of age. For large breed dogs, we recommend that spaying is delayed until 12 to 18 months of age (an additional fee of $145 will apply to dogs weighing >75lbs due to the added costs associated with increased patient size). There will also be an additional fee ($195 to $765) charged for patients that are in heat or pregnant at the time of the spay surgery. 5. The following procedures are not included in the Puppy Package, but can be performed while the pet is under general anesthesia for the neuter or spay procedure. All fees incurred for these additional services must be paid in full prior to the patient being discharged from the hospital. (Fees listed below are an estimation of cost and are subject to change.)

   1. microchipping with registration - additional fee of $105

   2. umbilical hernia repair (neuter) - additional fee of $275 to $370 (depending on severity of hernia)

   3. umbilical hernia repair (spay) - additional fee of $ 225 to $320 (depending on severity of hernia)

   4. retained deciduous teeth (baby teeth) extraction - additional fee of $65 and up (depending on the number of teeth)

   5. gastropexy (neuter) - additional fee of $1562 to $1942 (depending on patient size)

   6. gastropexy (spay) - additional fee of $1157 to $1397 (depending on patient size)

6. All vaccinations and well puppy exams included in the Puppy Package are for use in the initial puppy series only and cannot be used for 1 year adult boosters.

7. The Puppy Package and all of the included services will expire 18 months from the date of purchase as listed on page one of the agreement. Upon expiration, no refund will be issued for unused services.

8. If you choose to cancel the package for any reason, the package services that have been utilized will be assessed at full price and then deducted from the amount paid toward the package.

9. Puppy Packages are only valid at Eagle Animal Clinic and for the pet in which it was purchased. Unused services cannot be transferred to another pet and will only be honored for the pet listed on the agreement. However, services are transferable with the puppy (if you re-home the puppy, with your consent, the remaining package services will be transferred to the new owner).

10. Puppy Packages and included services are not eligible for additional discounting and cannot be combined with other special offers and promotions.
